We are required by law to safeguard your PHI, provide you with our privacy practices, and follow the terms of our current Notice of Privacy Practices. If a breach of unsecured PHI occurs, we will provide required notifications as applicable under law.

How to file a complaint
You may file a complaint directly with our Privacy Official using the contact information above. You may also file a complaint with the U.S. Department of Health & Human Services, Office for Civil Rights (OCR). You will not be penalized for filing a complaint.
